Do not use semaglutide if you or a family member have a history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Animal studies have shown a potential association between GLP-1 receptor agonists and thyroid tumors — the risk to humans is not yet fully established. Semaglutide is also not recommended if you have a personal history of pancreatitis.

Common side effects (especially during dose titration): nausea, vomiting, diarrhea, constipation, stomach discomfort. These are most common when starting or increasing your dose and typically improve over time. Anti-nausea medications (such as ondansetron) can help manage early side effects — discuss with your provider. Low blood sugar (hypoglycemia) risk increases if semaglutide is taken alongside sulfonylureas or insulin.
