Every other GLP-1 hits one receptor. Tirzepatide hits two.
Tirzepatide is a dual GLP-1 + GIP agonist — the first medication to activate two separate metabolic pathways simultaneously. While semaglutide targets only the GLP-1 receptor, tirzepatide’s dual mechanism produces more comprehensive effects on insulin signaling, appetite regulation, and fat metabolism.
Published clinical research has demonstrated greater weight reduction than any single-receptor GLP-1 therapy in the published literature. Beyond weight management, tirzepatide has shown significant improvements in obstructive sleep apnea severity — a finding the FDA recognized with a second approved indication for the molecule in 2024.
At Obsidian Genetics, we offer physician-supervised compounded tirzepatide — personalized dosing, flat monthly pricing, and a clinical team in your corner.

Why two receptors changes everything.
GLP-1 Receptor Activation
Like semaglutide, tirzepatide activates the GLP-1 receptor — reducing appetite, slowing gastric emptying, and improving insulin response. This alone drives significant weight loss.
GIP Receptor Activation
Unlike any other approved GLP-1 therapy, tirzepatide also activates the GIP receptor. GIP enhances fat burning, reduces GLP-1 side effects, and provides additional metabolic benefits.
The Dual Advantage
The dual-agonist mechanism produces weight loss outcomes no single-receptor GLP-1 has achieved. Two separate satiety signals. Two separate metabolic drivers. Measurably different results.
Tirzepatide vs. Semaglutide.
The head-to-head data.
In the 2025 SURMOUNT-5 trial — the first direct comparison of its kind — tirzepatide was tested head-to-head against semaglutide 2.4mg in people with obesity. Tirzepatide won. By a significant margin.

Safety & Considerations
Tirzepatide is a prescription medication. Your Obsidian Genetics provider will review your full health history before prescribing.
Gastrointestinal Effects
Nausea, vomiting, and diarrhea are most common during dose escalation. These typically resolve as your body adjusts. Your provider will titrate your dose gradually to minimize discomfort.
Hypoglycemia Risk
Low blood sugar is a rare risk with tirzepatide alone, but increases when combined with insulin or sulfonylureas. Your provider will account for any concurrent medications before prescribing.
Injection Site Reactions
Mild redness or discomfort at the injection site may occur. Rotating injection sites and using proper technique (detailed in your dosing guide) minimizes this risk.
⚠ Important Contraindications
Do not use tirzepatide if you or a family member have a personal history of medullary thyroid carcinoma (MTC) or Multiple Endocrine Neoplasia syndrome type 2 (MEN 2). Animal studies have shown a potential association between GLP-1 receptor agonists and thyroid tumors — the risk to humans is not fully established. Tirzepatide is also not recommended if you have a personal history of pancreatitis. Disclose your full medical history during your intake consultation. A prescription will only be issued if your provider determines tirzepatide is appropriate for you.
Frequently asked question
How is tirzepatide different from semaglutide?
Semaglutide is a GLP-1 receptor agonist — it targets one metabolic receptor. Tirzepatide is a dual GLP-1 + GIP agonist — it targets two. The GIP receptor activation provides additional appetite suppression, enhanced fat metabolism, and may reduce the nausea commonly associated with GLP-1-only therapies. In a 2025 head-to-head trial (SURMOUNT-5), tirzepatide produced 47% more weight loss than semaglutide 2.4mg.
How much weight can I expect to lose?
In the SURMOUNT-1 trial, participants on the highest dose of tirzepatide lost an average of 22.5% of their body weight over 72 weeks. 96% of all participants lost some weight, and 63% achieved at least 20% weight loss. Individual results vary based on starting weight, dose, adherence, and lifestyle. Your provider will set realistic expectations during your consultation.
What is compounded tirzepatide?
Compounded tirzepatide is tirzepatide prepared by a licensed 503B outsourcing facility — a federally regulated compounding pharmacy. It contains the same active ingredient as Mounjaro® and Zepbound® but is not manufactured by Eli Lilly. Compounded medications are not FDA-approved; however, they are legally prescribed and dispensed when produced by a licensed, regulated facility. All Obsidian Genetics medications come from licensed, inspected compounding pharmacies.
How is tirzepatide administered?
Tirzepatide is administered as a once-weekly subcutaneous injection (just under the skin). Common injection sites include the abdomen, thigh, or upper arm. Your plan includes syringes, alcohol prep pads, and a detailed dosing guide. Most patients find the injection simple and quick after the first few weeks.
Does my dose change over time — and does that affect my price?
Yes, tirzepatide is typically started at a low dose and titrated upward over several weeks to minimize side effects. Your provider manages this process. At Obsidian Genetics, your monthly price stays the same regardless of dose — we charge a flat rate based on the medication, not the dose level. No surprise charges.
What if I've been on semaglutide and want to switch?
Many patients transition from semaglutide to tirzepatide to achieve greater results. Your provider will review your current dose and history during the intake process. If a switch is appropriate, they’ll coordinate a dosing plan that accounts for your existing tolerance and minimizes side effects from the transition.
How should I store my tirzepatide?
Store tirzepatide in the refrigerator between 36°F and 46°F (2°C–8°C). Do not freeze. Keep out of direct sunlight. If your medication has been left at room temperature, contact your provider before using. Detailed storage instructions are included with each shipment.
